Murdostoun, Lanark, January 13th - Deceased was a surface labourer employed on a dirt bing after dark, which was lighted by 5 electric lamps fixed on wood poles. The lamps were connected in series between one phase and earthed neutral of a 440 volt 3-phase system. Insulated wires were carried by cross arms fixed at heights above the rail track, ranging from 7 ft. to 8 1/2 ft. These wires were within reach and were found broken, one wire being under the arm of deceased. The night was wet, and it is supposed that deceased took hold of the wires at a point where the insulation was defective and received a fatal shock. The pressure may have been 250 volts to earth. FAI record - NAS "Alexander Ellis, shale miner, 69 Glen Road, Livingston Station, died on 12 October 1920 in No. 5 Shale Mine, Deans, Bathgate Parish, Linlithgowshire, when he was electrocuted"
Deans No 5, Linlithgow, October 12th - Deceased was a Shale Miner, and received a fatal electric shock while about to enter a bothy on the surface, preparatory to going underground. A fellow worker within the bothy had, for a practical joke, connected the metal latch on the wooden door to the electric light switch, or fuse, within the room. The lamps were connected in series, between phases of a 440 volt 3-phase system with unearthed neutral, and each branch circuit was controlled by a single pole switch and fuse of domestic type. The lamp was not alight in the bothy at the time of the accident, sufficient illumination being obtained from a coal fire. Ardenrigg, Lanark, December 16th - Deceased was a Coal Cutting Machineman, and received a fatal electric shock from the bat, which was provided to connect the trailing cable to the electric coal cutter. The pressure of the circuit was 440 volts 3-phase and the neutral point was earthed through a small fuse and indicating lamp. The bat was mechanically defective, so that on withdrawal from the machine a brass nut dropped in such a way as to connect the metal case with a live terminal. The earthing circuit was also defective at the time of the accident.
