Please read the following before submitting this form
- All requests for additions to the site must be accompanied by details of the source, preferably including details from a death certificate. We cannot add names without a definite date of death (please see our FAQ for advice on how to obtain a Scottish death certificate).
- All information will be checked and confirmed new entries will be added to the site as soon as possible.
- You will be acknowledged as the source using the details entered under "Your name" - if you do not want to be acknowledged, please leave this field blank and an anonymous acknowledgement will be made.
- Please check the indexes on the site carefully before submitting information. We frequently get requests to add names which are already recorded. In many cases the Mine Inspector reports give the date of the accident, which may differ from the date of death.
